West County Health Centers shares how their rural location prompted staff to explore different ways to use “out-of-the-box” technology and software within the health center to improve their patients’ access to and experience of care, collaboration within care teams, and coordination across health care settings. As part of the project, staff began using secure instant messaging and video conferencing amongst each other and remote video conferencing with patients.
